Your journey begins with a consultation to assess your skin cancer diagnosis and determine if Mohs surgery is appropriate. We’ll discuss the procedure, answer your questions, and prepare you for the day of surgery.
On the day of surgery, layers of cancerous tissue are removed and examined under a microscope in real time. This process is repeated until all cancer cells are removed, ensuring complete removal and minimal impact on healthy tissue.
Once the cancer is removed, we perform any necessary reconstruction to restore your skin’s appearance. You’ll receive detailed aftercare instructions and schedule follow-ups to monitor healing and maintain long-term health.

Unless otherwise indicated, you should take all medications prescribed by your primary care physician prior to and on the morning of your procedure. This includes blood thinners such as Coumadin, Apixaban, Clopidogrel, Aspirin, etc.
However, you should avoid taking vitamins or herbal supplements that can thin the blood such as fish oil, vitamin E, Ginkgo biloba, garlic, turmeric, feverfew or over-the-counter anti-inflammatory medications such as Advil or Aleve.
Plan to eat breakfast the morning of your procedure and bring any medications or medical devices such as oxygen you may require throughout your time spent with us. It is also encouraged to bring something to read or work on during the tissue processing time. Our office is equipped with music and we offer light snacks and beverages.
Mohs surgery is performed under local anesthesia, so you’ll remain comfortable throughout the procedure. Some patients may experience swelling and/or bruising around the procedure site along with mild discomfort or tenderness during recovery, which can be managed with over-the-counter pain relief.
The length of the procedure depends on the size, location, and severity of the skin cancer. We cannot estimate in advance how many layers will be needed to successfully remove the skin cancer and each case is unique. You should expect to spend a minimum of 4 hours at our office.
During your visit, a portion of that time will be surgical while the other aspects include preparation, mapping and color-coding each layer of skin removed, processing the tissue to create a microscope slide which typically takes 45 minutes per layer, and analysis of the skin layer under the microscope by Dr. Lazzara.
Once clear, Dr. Lazzara will review the best plan for healing whether that be stitching the site in a straight line, performing a flap or graft, or letting the body heal itself. Some cases may require referral to a plastic surgeon or ear, nose, and throat (ENT) surgeon for the best healing. After executing the reconstruction, a pressure bandage will be placed to protect the surgical site which will remain in place and dry for a minimum of 48 hours. Wound care instructions will be reviewed in detail and any questions answered.
After the procedure, it is normal to experience soreness/mild discomfort, swelling and bruising at the surgical site or surrounding area. When removing the skin cancer superficial nerves can be affected, resulting in temporary numbness or tingling in the area of your surgical site; this is particularly true when working on the forehead or scalp. Numbness typically resolves over time.
You will be informed to always keep the surgical site moisturized with a bland ointment like Aquaphor or Vaseline and covered with a bandage until follow-up appointment. If healing involves stitches, you should expect sutures to remain for 7-12 days. Surgical scars take 3-12 months to fully heal and look their best.
We will counsel you on care you can perform at home to obtain optimal scar appearance including firm massaging following 3-4 weeks post-op, use of scar gels, and sun protection. You will be informed to always keep surgical site moisturized with a bland ointment like Aquaphor or Vaseline and covered with a bandage until follow-up appointment. If healing involves stitches, you should expect sutures to remain for 7-12 days. Surgical scars take 3-12 months to fully heal and look their best.
Our team will place a clean bandage on your surgical site which should remain dry and intact for a minimum of 48 hours. A wound care sheet will be provided and will highlight any deviations from this standard instruction such as keeping your bandage in place for 72 hours or 1 week (in the case of a graft), taking postoperative antibiotics if required, and when to return to our office for follow-up.
Following a minimum of 48 hours, you can allow the bandage to become soapy wet for ease of removal. Gently clean your surgical site with mild soap and water (i.e. Dove) or hydrogen peroxide. After pat drying the area, apply a generous coating of Vaseline or Aquaphor and re-cover with a bandage. The best materials to use is a non-stick gauze pad and paper tape. If your surgery involved your leg, an additional pressure bandage such as compression stocking or Coban wrap is advised.
Remember to rest and elevate your surgical site. It is also important to avoid strenuous activity and bending head first such as when you put your shoes on. Ice can also be applied intermittently for 20 minutes on and then 20 minutes off. Ice should be wrapped in a cloth or towel and placed directly on top of your bandage protecting your surgical site.

At Lazzara Dermatology we take the utmost care to prevent surgical complications such as bleeding. However, even with taking every proactive measure, you may encounter bleeding post-operatively.
You are at particular risk of bleeding if you take blood thinning medications, have a blood clotting disorder, or perform too much activity following your surgery. If you begin to bleed at your surgical site, you should apply firm pressure with ice wrapped in a towel to your surgical site bandage. Do not lift pressure or peak to see if the area is still bleeding for 20 minutes.
If you continue to bleed following the 20 minutes of held pressure, repeat for another 20 minutes. If you are still bleeding at that point, give us a call.
